Application & Training Agreemen / FAA Type Rating 8710 Application Worksheet

NOTE: You will be required to fill out two applications; the Application & Training Agreement and
the FAA Type Rating Worksheet. Every applicant must fill out both forms in order. Once you have
completed the CPT Application & Training Agreement you will be sent an email with a list of all
the documents that need to be submitted to Crew Pilot Training.
