VA attachment #4: Name, licenses # COMM. or ATP, address, city, state, zip, class of medical and date issued. Note: If you will be updating your medical before class, send in a copy of the old medical with a note saying you will be getting a new medical before your class begins (YOU MUST HAVE A CURRENT FIRST CLASS MEDICAL BEFORE YOUR CLASS STARTS). Sign where indicated.

VA attachment #5: Initial all blanks and sign where indicated.

VA FORM 22-1990 (application for education benefits): Follow instructions attached to this form. PLEASE DO NOT SEPARATE FORM. (MAKE A COPY OF THIS FORM AND KEEP IN SAFE PLACE).

VA FORM 22-1999 (enrollment certification): COMPLETE SIDE "B" ONLY. Fill in the following blanks, 1, 2 (if known), 3 and 4, "NO SIGNATURE REQUIRED". PLEASE DO NOT SEPARATE THIS FORM. Crew Pilot Training will complete the balance of form. (WE SUGGEST MAKING A COPY OF THIS AND KEEPING IT IN A SAFE PLACE)

VA FORM 22-6553c (monthly certification of flight training): Fill in the following blanks. 1,2 (S/S#), 4, 11B (ONLY IF YOUR MEDICAL IS CURRENT, 13A. DO NOT DATE. THIS IS A COMPLETION FORM AND THE SCHOOL WILL CERTIFY THE FORM AND DATE ONCE YOU HAVE GRADUATED, BEFORE FILING WITH THE VA FOR YOU. THIS FORM IS THE MOST IMPORTANT FORM AND IS USED FOR YOUR VA REFUND. MAKE A COPY AND KEEP IN A SAFE PLACE.
